Essential Duties and Responsibilities
Client Assessment and Support
- Meet and Interview Clients: Conduct assessments to understand ongoing needs and match available state and federal programs to address unmet needs.
- Develop Client Profiles: Manage information input to the central case management system, reflecting updates and monitoring progress.
- Coordinate with Agencies: Work with agencies like FEMA, USDA, and NM Human Services Department to coordinate programmatic solutions.
- Provide Guidance and Support: Offer compassionate communication and support through various channels, including in-person, phone, and email.
Documentation and Reporting
- Complete Case Notes and Records: Maintain accurate and timely case notes, files, and records using appropriate tracking and database systems.
- Generate Reports: Regularly report on client progress, ensuring compliance with federal, state, and local funding guidelines.
Participation and Compliance
- Staff Meetings and Trainings: Participate in all required meetings, trainings, and educational workshops.
- Compliance with Guidelines: Ensure all activities comply with program guidelines and regulatory requirements.
